CFHS Information & Well-Child Clinics

The Child and Family Health Services (CHFS) program is designed as an organized community effort that develops programs to improve the health status of women and children in Ohio.

The goal of the CFHS Grant Program is to eliminate health deparities, improve birth outcomes, and improve the health status of women, infants and children in Ohio.

 

CFHS Funded Agencies are made possible through a grant from the Ohio Department of Health in the interest of healthy children in cooperation with the Lead Agency, the Ashtabula County Health Department with services subcontracted with the Ashtabula City Health Department, the Conneaut City Health Department.

Ashtabula County Well Child Clinic

Well Child Exams Include:
• Health history
• Physical assessment
• Nutrition assessment
• Physical growth and development evaluation
• Vision, hearing and speech screening
• Blood tests for Anemia and Lead Poisoning
• Urine and TB testing
• Health and safety education
• Assistance with referrals for identified needs/problems

Complete physicals up to 21 years of age
